Human Health Medical Centres Provide Government-Subsidized Services under the Chronic Disease Co-Care Pilot Scheme
1. What problems can the “Three Highs” cause? Who is at risk?
According to the latest Population Health Survey by the Department of Health*, nearly half of Hong Kong residents aged 15–84 suffer from at least one of the “Three Highs” — high blood pressure, high blood sugar, or high cholesterol. These conditions can lead to serious diseases such as stroke or kidney failure and should never be ignored.
Elderly people, smokers, and those who are overweight fall into high-risk categories. It is strongly recommended that they undergo regular health checks and monitor relevant indicators to detect problems early and prevent disease progression.
2. Who can join the Chronic Disease Co-Care Pilot Scheme?

Hong Kong residents aged 45 or above

Those with no known diabetes or hypertension

Those who agree to join the Electronic Health Record Sharing System (eHealth)
The government launched the Chronic Disease Co-Care Pilot Scheme (CDCC) on 13 November 2023. Eligible individuals can enrol through any District Health Centre (DHC) or DHC Express, where they will be matched with a family doctor for diabetes and hypertension screening and long-term follow-up.
3. Do participants need to pay?
Under the CDCC Scheme, participants share a portion of the cost (co-payment model). With government subsidy, they receive diabetes and hypertension screening, medical consultations, laboratory testing, and — if clinically indicated — prescribed medication, as well as follow-up by nurses and allied health professionals.
Participants diagnosed with borderline high blood sugar, diabetes, or hypertension will receive further government-subsidized management from their chosen family doctor:

Individuals with borderline high blood sugar may receive up to 4 subsidized consultations per year.

Patients with diabetes and/or hypertension may receive up to 6 subsidized consultations per year.

For medications listed under the CDCC essential drug formulary, participants do not need to pay additional drug costs.
4. How much subsidy is provided under the scheme?
The programme is divided into two phases: Screening Stage and Treatment Stage.

Screening Stage:
The government provides a one-off subsidy of HK$196. Participants only need to pay a co-payment of HK$120 for the screening consultation and related tests. -
Treatment Stage:
The government subsidizes HK$166 per consultation, and participants pay only the co-payment amount set by their family doctor when joining the scheme.
Medications covered under the Designated Drug List and up to three days of acute illness medication are fully subsidized.

Doctors may charge an additional fee for services outside the scheme, subject to prior agreement with the patient.
The government also provides partial subsidies for laboratory tests, nurse clinic services, and allied health support during the treatment stage. Participants only need to pay the corresponding co-payment amount.
Eligible persons may also use their Elderly Health Care Vouchers to cover the co-payment. The CDCC is included as a designated primary healthcare service under the Elderly Health Care Voucher Pilot Reward Scheme (learn more).
5. How can I join the government-subsidized Chronic Disease Co-Care Pilot Scheme?
General practitioners under Human Health are registered service providers for the scheme. As a government-recognized provider, any Human Health Medical Centre can offer CDCC services to eligible Hong Kong residents, making it convenient for the public to access subsidized healthcare.
