According to the latest Population Health Survey data from the Department of Health, nearly half of the Hong Kong population aged 15 to 84 have at least one of the "three highs" (hypertension, high blood sugar, high cholesterol) health problems. The "three highs" can lead to serious diseases like stroke or kidney disease, and should not be overlooked. The elderly, smokers and obese individuals are high-risk groups for the "three highs", so regular check-ups and monitoring of relevant indicators are recommended to make early improvements and prevent worsening of the problems.

 

The government's "Chronic Disease Co-management Pilot Scheme" was officially launched on November 13, 2023. Hong Kong residents aged 45 or above without known history of diabetes or hypertension can participate in the scheme through any of the District Health Centres/Stations from that day, and be paired with a family doctor for diabetes and hypertension screening and long-term follow-up.

 

Under the Co-management Scheme, eligible persons will co-pay a certain amount and receive government subsidies to undergo diabetes and hypertension screening, doctor consultations and laboratory tests in the private healthcare market, as well as prescribed medications based on their health conditions, and follow-up services at nurse clinics and allied health services.

 

If participants are diagnosed with pre-diabetes, or already have diabetes or hypertension, they will receive government subsidies and be followed up long-term by their selected family doctors. Pre-diabetic persons can receive up to 4 subsidized consultations per year; while those with diabetes and/or hypertension can receive up to 6 subsidized consultations per year. Participants prescribed with the "Co-management Scheme" basic medicines after consultations will not need to pay any additional medication fees.
